How can I determine what index this is referring to and how do I repair it?
To
hopefully fix it for now reindex all the attributes on both servers.
Use db2index.pl, and do not specify an attribute -> this will reindex
all the attributes.

389-Directory/1.2.11.15 B2015.345.187 (389-ds-base-1.2.11.15-69.el6_7.x86_64 is the RPM)
