OK, I think I largely understand what needs fixing. The first and most
obvious one is that kde index.docbook files need to start with
<!DOCTYPE book PUBLIC "-//KDE//DTD DocBook XML V4.2-Based Variant ...
instead of
<!DOCTYPE book PUBLIC "-//KDE//DTD DocBook XML V4.1.2-Based Variant ...
then there are sometimes other minor changes required to comply with the
slightly stricter dtd.

K.. I see that things are rather extensive. Its 23:10 right now. I will need
to go over this when my head is a bit more awake.
Any pointers you could provide would be good. And how will the proposed
changes effect f13, f12 and possibly f11 builds?
