Regarding profile registered profile

Hi, I read profile registration profile standard document. I don't have clear about it. If you can, please tell about. If we have created a new profile, should need to register it right? How it achieve? Please provide the steps for it... Also its implementation samples.. That's may be very helpful for me. I am waiting for your response. Regards, George
