Hi Clark,
could it be that the ECKD DASDs contain some existing broken Linux Disk Layout leading to the I/O errors? If so, I'd recommend performing a new manual low-level format of those disks. FYI and not related to the I/O errors: according to the z/VM "CP Commands and Utilities Reference" and your device number query (which corresponds to the output format of CP QUERY VIRTUAL DASD), you seem to have in-memory VDISKs which are FBA (9336) on devno 0300 and 0301. AFAIK, anaconda does not handle FBA DASDs, see also https://bugzilla.redhat.com/show_bug.cgi?id=601855 and https://bugzilla.redhat.com/show_bug.cgi?id=707026.

Looks like hardware issues to me. The kernel is not happy. This block is recurring through your log file.
